Community Habilitation Worker
Join our team and make a difference! As a Community Habilitation Worker, you’ll empower individuals with developmental disabilities by promoting independence, personal choice, and community integration.
Key Responsibilities
- Provide person-centered support in homes and the community.
- Assist individuals in building life skills, relationships, and self-advocacy.
- Ensure safety, well-being, and adherence to individual care plans.
- Encourage community involvement and inclusion.
- Maintain accurate documentation and uphold compliance standards.
- Communicate effectively with individuals, families, and team members.
- Participate in training and professional development.
Qualifications
- High School Diploma or GED.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills for documentation and communication.
- Valid driver’s license preferred.
- Bilingual in English & Bengali is a plus!
Work Environment & Physical Demands
- Community-based and home settings.
- Ability to perform physical tasks such as standing, lifting, and walking.
