Pay Rate: $22.00 per hour

Hours: 40 hours/5 days a week, 10 AM -7 PM (with 1 hour lunch). Must be available for Weekends

 

OVERVIEW OF PRIMARY RESPONSIBILITES:

Reporting to the IDD Program Director, the Community Habilitation Lead is responsible for developing and implementing programming that is challenging, inspiring, and aligned with the valued outcomes, supports and safeguards of the individuals who receive services. Provide oversight and programming that is person-centered, fosters community integration, and focuses on skill acquisition and, or retention while remaining compliant to agency, state, and federal requirements.

 

PRIMARY FUNCTIONS:

  • Works with IDD Program Director and Community Habilitation Supervisor to develop, coordinate and implement program goals.
  • Conduct visitations to staff in the home and community setting to provide support, role model, and coach staff with the learning and implementation of the Staff Action Plan, service documentation and protocols as needed. 
  • Ensures that program records are up-to-date and current for each person enrolled in the service.
  • Works with Care Coordinators to obtain and share documents to ensure complete and contemporaneous files.
  • Prepares for and attends Life Plan and other team meetings, and utilizes the information gathered for the development of the Staff Action Plan.  
  • Completes match up with newly hired staff and individuals seeking services, while considering individual’s specific interests, challenges, needs and supports.
  • Communicate any training needs to the Community Habilitation Supervisor and/or IDD Program Director.
  • Monitors the electronic health record (i.e., eVero) to ensure complete, accurate and timely complementation of service delivery as required.
  • Monitors service delivery as required to ensure quality and adherence to NADSP Core Competencies, Code of Ethics, Staff Action Plan and Safeguards. Take corrective actions as necessary and immediately notify Supervisor/Assistant Director of concerns.
  • Communicate professionally with persons receiving services, families, team members, and the community.
  • Creates and maintains a healthy and productive work environment by modeling positive behaviors that teach patience, professionalism, and teamwork.
  • Follow and adhere to all agency and program policy and procedures, including HIPAA, Confidentiality, Accident / Incident Reporting, implementing the Corporate Compliance Plan and related policies and procedures, and reporting program needs to the Supervisor and/or Program Director. 
  • Actively participate in program meetings and staff meetings on a routine basis to ensure timely communication with the team.
  • Report to work promptly and adhere to the assigned schedule.
  • Complete all required initial and annual training as well as ongoing training to increase knowledge.
  • Actively participate in supervision for professional growth and development.
  • Maintains a valid NYS driver’s license in good standing and operates agency vehicles as needed.
  • Perform other related duties as requested/assigned.
  • Utilize personal vehicle vehicle and/or reliable means on transportation to conduct home visitation across the New York City region.

QUALIFICATIONS AND ATTRIBUTES

EDUCATION and/or EXPERIENCE

  • Bachelor’s degree in Psychology or related human services field preferred.
  • At least 3 years’ experience working with people with special needs; a minimum of 1 year at a managerial level preferred.
  • Working knowledge of OPWDD regulations and payment standards as they pertain to Community Habilitation, and Respite.
