Hours: M-F 830 am to 5 pm

Salary: $80,000

OVERVIEW OF PRIMARY RESPONSIBILITES:
 
The OnTrackNY Program is an innovative treatment program.  The program serves adolescents and young adults who are within two years of experiencing the onset of non-affective psychosis. 
The Team Leader will work 1.0 FTE as part of a small multidisciplinary team providing coordinated and assertive treatment in the earliest stages of psychosis.
 
 
PRIMARY FUNCTIONS:
 
  • Provides overall supervision to the team and is responsible for allocation of staff time. 
  • Ensures staff attendance at required OnTrack NY trainings and consultation calls, and completion of additional OnTrackNY related training modules in the Center for Practice Innovation’s Learning Management System (LMS) 
  • Fosters and models principles that underlie the model (recovery, shared decision making/duty to care-dignity of risk, person centered, cultural humility)
  • Ensures all team members are kept informed of issues involving participants and their families. 
  • Ensures coverage of 24/7 call schedule and access to team.
  • Ensures team members can deliver services in the community (off-site) as needed based on participant and family preferences.
  • Monitors care load size and coordinates program intakes and discharges. 
  • Facilitates weekly team meetings and provides training and individual supervision to team Primary Clinicians, Supported Employment and Education Specialist, and Peer Specialist; and provides administrative supervision to the Prescriber and Nurse.
  • Serves as liaison between team and (host agency name here) to ensure adherence to the coordinated specialty care model in the contest of agency name protocols and policies.
  • Serves as the Primary Clinician for a subset of program participants.  In this role of Primary Clinician, the Team Leader will:
    • Serve as the point person for the participant and their family;
    • Provide services in the community (off-site) as needed based on participant and family preferences.
    • Conduct needs assessments;
    • Engage in safety planning and wellness management planning with participants;
    • Connect participants and families with the services and supports they need;
    • Provide behavioral interventions and take an especially active role in the family support domain.
    • Conduct clinical assessment to determine the participant’s level of social functioning and substance use via a shared decision making (SDM) framework
    • Provide Social Skills Training, Substance Abuse Treatment, Behavioral Activation, Coping Skills Training, and Psycho-education.
    • Provide psychoeducation regarding first episode psychosis (FEP) and the goals of the OnTrackNY program;
    • Provide supportive therapy;
    • Develop and modify treatment using a shared decision-making process with clients
    • Work with participants and family for transition from the team to other services and supports, and ensure stable links at discharge.
 
QUALIFICATIONS AND ATTRIBUTES:
 
  • Experience working with adolescents and young adults;
  • Experience in community-based mental health services.
 
EDUCATION AND EXPERIENCE:
 
  • Advanced clinical license (social work, psychology);   LCSW/LMHC
