Hours Mon-Fri Days (Flexibility needed)

We are seeking a dedicated Transitional Living Counselor (TLC) to join our Mental Health Housing and Recovery Program. This is a rewarding opportunity to make a difference in the lives of individuals on their recovery journey while working as part of a professional, supportive team.

As a TLC, you will provide direct support to individuals transitioning toward independent living. You will:

- Perform assessments and develop service plans
- Provide restorative and recovery-focused services
- Teach habilitation and life skills that foster independence
- Assist individuals in navigating community resources and service systems
- Maintain timely and accurate documentation in compliance with NYS OMH and Medicaid regulations
Each counselor will manage a caseload of 8–9 individuals and will participate in a rotating on-call schedule.
What We’re Looking For:
The ideal candidate is enthusiastic, organized, and compassionate, with strong communication and computer skills. A background in human services and/or lived recovery experience is highly valued.

Qualifications:
- High School diploma/GED plus 3 years’ experience in human services; OR
- Associate’s degree in human services plus 1–2 years’ experience; OR
- Bachelor’s or Master’s degree in human services (experience preferred)
