Position: Hospice-Bereavement Coordinator
Reports to: Director of Patient Care Services
As a Hospice-Bereavement Coordinator, you will:
- Coordinate the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
- Plan for Bereavement Program development, expansion and refinement through annual evaluation of services.
- Provide support to hospice personnel coping with work related grief.
- Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups).
- Function independently, but seek and accept guidance from other members of the interdisciplinary group, or from members of the community.
- Participate in hospice activities such as inservice education, hospice personnel meetings, and relevant committees.
- Advance professional knowledge and skills.
- Actively participate in quality assessment performance improvement teams and activities.
- Accept and perform other related duties and responsibilities as required.
- Provide bereavement information and referral services to callers from the community.
- Ensure the accuracy of public information materials
- Expand and maintain community resource information as needed.
- Interact effectively with managers, employees, clients and professional colleagues.
- Comply with applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.
- Participate in required orientation and periodic retraining related to the Compliance Program: Code of Conduct, Documentation Standards and HIPAA.
- Report concerns and suspected incidences of non-compliance using the AccentCare Reporting Process.
- Cooperate with monitoring and audit functions and investigations.
- Participate, as requested, in process improvement activities.
Requirements of Qualified Candidates:
- Master’s degree, in the area of mental health, preferred or Bachelor’s degree, in counseling, theology or related field required. Experience with families/caregivers and patients through death/dying issues
- Knowledge and commitment to hospice philosophy
- Demonstrates good verbal and written communication, and organization skills
- Understanding of grief response and experience with counseling individuals and/or in groups regarding loss
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
- Must successfully complete an introductory period. This introductory period is to ensure the aide is competent in the above qualifications, and is documented by the supervisor and the employee
