Position:           Assistant Coordinator

Reports to:        Patient Care Coordinator

Job Description/Requirements:                                                                                                                          

As an Assistant Coordinator, you will:

  • Bilingual Assistant Coordinator is responsible for assisting the Patient Care Coordinators with intake calls from insurance plans, inputting new cases and scheduling patient care services.
  • Conduct Introduction calls to new members and providing efficient customer service.
  • Accept or reject cases based on patient needs, availability of skill sets time and/or location and monitors and navigates the Home Health Aide Scheduling Systems.
  • Handle intake calls from insurance plans.
  • Input new case/schedule into Scheduling systems.
  • Monitor/Navigate our Scheduling systems.
  • Responsible for working with client intakes and assessments from providers.
  • Accepts or reject cases based on patient needs availability of skill sets, time and/or location and monitors and navigates the Scheduling Systems.

Requirements of Qualified Candidates:

  • Bilingual in English and Spanish required
  • High School diploma/GED required; Associates degree preferred.
  • Minimum of one (1) years related experience in a health care/social service environment preferred.
  • Must have working knowledge of Microsoft Office.
  • Excellent written and interpersonal communication skills required.
