University Medical Partners (UMP), a partner of Stanford Medicine, is seeking a full-time LCSW or LPC (Licensed Professional Counselor) to join our clinic network.

Position Details:

  • Seeking full-time LCSW or LPC (Licensed Professional Counselor)
  • Clinic based: Hybrid of virtual video-therapy and in-person visits
  • Work closely with consultant Psychiatrist and Primary Care Providers in a collaborative care delivery model
  • Minimum 3 years’ of recent behavioral health experience required
  • Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement, education, and follow-up care
  • Provide brief behavioral interventions using evidence-based modalities such as behavioral activation, problem-solving treatment, motivation interviewing, or other treatments as appropriate
  • Support psychotropic medication management as prescribed by medical provides, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment
  • Systematically track treatment response and monitor patients (in person or by video) for changes in clinical symptoms, treatment side effects or complications.
  • Average case load of 75-100 patients
  • Regular business hours; no call
  • Other minimum qualifications include:
    • Active California License
    • Outstanding clinical, collaborative and communication skills in a team setting
    • Ability to work in fast-paced clinical setting
  • Additional preferences:
    • Familiarity working with Epic EMR
    • Experience working in multi-specialty group and/or integrated healthcare system
    • Experience providing behavioral health interventions in a primary care setting
