Role Description:

Oak Street Health is attracting and cultivating team members who embody our values and are passionate about the mission to rebuild healthcare as it should be.  Our focus includes Primary Care, Podiatry, and Behavioral Health.  Podiatry provides essential care for our patient population--especially for diabetics.

We start simple--build meaningful relationships with patients and their families and then layer on resources to help our teams produce excellent care.  Our physicians practice with a full team behind them which is made up of the following:

  • Registered Nurse

  • Medical Assistant

  • Clinical Informatics Specialist

Our facilities are all new and designed around this model of comprehensive, personalized primary care.  We use technology to improve the quality of care and efficiency of our providers.  Oak Street Health is the future of medicine where care is coordinated thoughtfully and providers are rewarded for personalized care.
