Role Description:

The Kidney Care Program Medical Director will join Oak Street at a critical time in the history of our company and of primary care. The opportunity for impact is large and growing, and Oak Street is leading the transition from fee-for-service to value-based care. The Medical Director will be responsible, with their dyad - the Kidney Care Program Director, Operations - for helping create and lead a new Kidney Care Program at Oak Street to provide high-touch, specialized renal care services to late stage chronic kidney disease (CKD) and end-stage renal disease (ESRD) patients to reduce their medical costs and improve health outcomes. The Medical Director will manage multiple kidney care teams across select markets, conduct an initial intake visit with each CKD / ESRD patient enrolled in the program, and provide guidance on any complex CKD / ESRD patient cases. The Medical Director will also work in close partnership with a central Kidney Care Program operations team and the Population Health team to establish the program's clinical guidelines, practices, and KPIs.



 

Core Responsibilities:

  • Overall Program Success: The Kidney Care Program Medical Director will play a critical role in establishing a new Kidney Care Program at Oak Street Health. A strong candidate will have a demonstrated track record not only of excellent patient care but also leading successful, innovative programs within healthcare organizations.

  • Kidney Care Program Clinical Design: The Program Medical Director will lead the clinical design process in close partnership with the Director of Program Operations and the Population Health team. 

  • Provider Management: The Medical Director will manage providers & other patient-facing employees involved in the program as a part of the Kidney Care Team. The Medical Director will answer complex care questions from the Kidney Care Team, provide professional mentorship, and establish strong working relationships with other medical directors within Oak Street Health.

  • CKD / ESRD Patient Care: The Medical Director will spend part of their time with patients, supporting care plan development for CKD / ESRD patients, managing more complex patient cases, and coordinating with the Oak Street PCP regarding non-kidney comorbidities.

  • Payer & Provider Relationships: The Medical Director will also play a role in conversations with external partners as the clinical leader of the program. This may include clinician-to-clinician conversations with specific specialists (e.g. vascular surgeons, general surgeons, and interventional radiologists) and payer partners. 


 

What are we looking for?

  • Extensive experience in clinical leadership roles, including leading and coaching providers and owning specific KPIs and/or financial metrics tied to program success

  • Demonstrated ability to work & communicate cross-functionally with providers, administrators, operators, and strategists

  • Aware of their leadership skills and potential for impact and eager to share those with a fast growing, energetic team on the leading edge of healthcare innovation

  • M.D. graduate

  • Board Certification in Nephrology is preferred, but Internists or Family Physicians with the right experience and skills will also be considered

  • Additional fellowships welcomed

  • 5-15 years of experience in clinical experience

  • Experience using a metrics-driven approach to analyze cost, quality, and satisfaction date to drive clinical strategy and program redesign
