Nashville General Hospital is hiring for a Licensed Clinical Social Worker for Patient Centered Medical Home as a full-time job in Nashville, Tennessee.
The Licensed Clinical Social Worker (LCSW) is a specialist in mental health services with broad knowledge and experience in the treatment of mental, behavioral, and emotional disorders of individuals, families, and groups. The LCSW works with patients to assess their personal situation both in terms of internal situation and external factors such as economic, societal, home, and career in order to create an inventory of both strengths and perceived weaknesses or obstacles.
Licensed Clinical Social Worker Roles and Responsibilities:
- Coordinates and provides necessary clinical updates and reviews with medical provider services in order to insure appropriate intervention, and monitoring of social work service plans in areas of personal, social, or economic resources, conditions, or problems.
- Provides psychosocial assessment, diagnostic impressions, treatment of individuals/couples/families/groups under the supervision of psychiatry and/or medical physicians, prevention of psychosocial dysfunction/disability/impairment, including emotional/mental/behavioral disorders, and evaluation of practice effectiveness.
- Plans and maintains referral and coordination services with health and social service agencies to provide optimal patient care.
- Conducts individual counseling on patients in crisis and/or based on physician referrals.
- Formulates a plan of care acceptable to patient, family and health care team.
- Responds to referrals and begin care planning in a timely manner as defined by departmental policies.
- Participate in daily care team huddles.
- Participates in ongoing training in adult behavioral health issues and provides training to primary care clinic staff in mental health topics including counseling skills and confidentiality, medication use and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
- Serve as an expert/resource for available community resources, applicable to identified patient needs.
- Educate patient and families regarding care plan and accessing community resources.
- Maintain appropriate documentation of services in client electronic record utilizing an electronic health record (EHR) format. Document according to departmental standards, Federal and State regulations.
- Responsible for continuity of care in the provision of comprehensive services and coordinates communication with other service components of the clinic and outside agencies involved with agency clients.
- Participates in quality assurance activities for the programs service delivery, including clinical care and documentation.
- Assists with administration, development, implementation, and evaluation of social work programs and policies.
Required Skills & Qualifications:
- Master’s Degree in Counseling or Social Work required.
- Clinical License in Social Work required.
- 3-5 years experience in field required.
- Experience working with adult population required.
