Nashville General Hospital is hiring an Ambulatory Navigator for our Clinics for Full-time day! 
   
We are seeking a compassionate and resourceful Navigator to join our ambulatory primary care and specialty clinic. The ideal candidate will assist patients in navigating the healthcare system, ensuring they receive coordinated, accessible, and patient-centered care within a Patient-Centered Medical Home (PCMH) model. This role involves direct patient interaction, care coordination, and collaboration with the healthcare team to enhance patient outcomes and experience.
As an Ambulatory Navigator, you will: 
  • Assist patients in understanding their diagnoses, treatment options, and care plans
  • Educate patients on managing chronic conditions, preventive care, and healthy lifestyle choices
  • Coordinate patient appointments, referrals, and follow-up visits, ensuring seamless transitions between providers and services
  • Collaborate with healthcare providers, specialists, and other care team members to develop and implement comprehensive care plans
  • Utilize health information technology to track patient outcomes, identify care gaps, and implement evidence-based practices
  • Connect patients with community resources, social services, and support groups as needed
  • Assist patients with financial and insurance issues, including obtaining pre-authorizations and understanding coverage options
  • Provide information on available healthcare services, programs, and resources within and outside the clinic
 
Required Skills & Qualifications:  
  • Associate’s degree in healthcare, social work, or a related field preferred.
  • Minimum of 2 years of experience in patient navigation, care coordination, or a similar role in a healthcare setting.
  • License to practice as a Registered Nurse in the State of Tennessee preferred, but not required
