Nashville General Hospital is hiring a Nurse Coordinator - Community Care for full-time Days!  
    
This position provides an integral role as an interdisciplinary team whose purpose is to improve the ability of patients designated as moderate and high risk to maintain and improve their health and avoid preventable emergency department visits and hospitalizations.  The CCT Nurse Coordinator, with the assistance of the CCT multidisciplinary team, will ensure continuity and coordination of care across transitions, including hospital/community facility to home; will identify barriers and facilitate optimal care delivery, including addressing gaps, medical, pharmaceutical, and social needs; and will develop a comprehensive care plan to address those needs that include prevention, patient self-management, access to community resources and education about their chronic disease condition. *On-call and weekend coverage rotation*  
  
As a Nurse Coordinator, you will:   
  • Assist in the management of patients with chronic diseases following established protocols and systems for disease management in collaboration with providers.
  • Conduct home visits to assess patient needs, implement care plans, and follow up on patients
  • Create and maintain schedules for clinicians
  • Provides telephonic outreach and in-person consultation to members identified as medium and high risk, having a chronic condition(s) and in need of preventive services.
  • Collects in-depth health information about a member’s health history as well as assessing a member’s individual needs to assist in identifying higher-risk members and develop a comprehensive plan of care addressing these needs.
  • Provide patient and family education on prevention, early detection of exacerbations of chronic disease and development and implementation of a chronic disease action plan.
  • Conducts comprehensive nursing coverage for triage and assessments including medical, behavioral, and social needs of the patient identifying services patient has received, reasons for prior admissions and any gaps in care based on evidence based clinical standards and communicate findings with the patient's primary care physician.
Required Skills & Qualifications:   
  • Active license to practice as a practical or registered nurse in the state of Tennessee
  • Bachelor’s degree in Nursing preferred
  • 3 years of Clinical Experience preferred
  • BLS certification required
