Description

The Social Worker (LMSW) is a professional, who collaborates with a multi-disciplinary team, inclusive but not limited to physicians, care managers, pharmacy, nursing and mid-level providers to coordinate care for geriatric patients and their families. Provides various social work services to meet needs of patients and family members, with a primary focus of providing therapeutic services. Services may include psychosocial evaluation of patient and relationship to family members in considering patient medical needs and prognosis. Provides crisis intervention as necessary to patient and family members. Is responsible for development and facilitation of therapeutic support groups. Social work practices and services are consistent with applicable accreditation and licensing standards, and reflect the National Association of Social Worker Code of Ethics.
