RN Post-Acute Care Coordinator - Full Time

Under the direction of the Director of Medical Management, in cooperation and collaboration with Care Management Teams across the continuum, the Post-Acute Care Coordinator provides transitional care assistance, coordination of continuing care, education, advocacy, and provides staff, providers and facilities with necessary resources to facilitate the coordination and receipt of patient care at the appropriate level within the community, in accordance with Federal, State and local standards, guidelines and regulations, established policies and procedures, as directed by the Director of Care Management /Social Work/designee, to ensure adherence to the highest degree of quality care.

Education
Minimum: Bachelor's of Science Degree in Nursing; or Master's in Social Work from an accredited college or university.
Credentials/Licensure
Minimum: Current, active licensure as a Registered Nurse or Social Worker in the State of Michigan.
Related Experience
Minimum: Three years of acute/sub-acute clinical experience. Two years of case management experience.
Preferred: Experience working in a Skilled Nursing Facility or Home Health environment. Familiarity with relevant governmental and payer regulations related to the post-acute care environment.

Ability to plan, organize, develop, implement and interpret organizational programs, goals, objectives, policies and procedures. Ability to adapt to ongoing change and work in a fast-paced, customer driven environment. Self-starter, highly motivated with high energy level. Ability to type with speed and accuracy.
Computer Competency
Familiarity with standard desktop and windows based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.
Physical/Mental
Minimum: Ability to see, hear and smell or use appropriate tools to meet position requirements. Ability to speak, read and write English comprehensibly. Ability to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile individuals within the community. Ability to deal tactfully with staff, residents, family members, visitors, government agencies and the general public.
