Description

• M-F, 9:00am – 3:00pm
• Opportunities for additional evening and weekend hours available if desired

• Provide direct care and assist assigned individuals with accessing the community and increasing independence with communication, socialization and daily living skills
• Implement individual’s care plans and provide ongoing feedback regarding additional or changing care plan needs to child’s supervisor or Day Habilitation Supervisor
• Required to become certified for medication administration, seizures, and g-tube feedings as well as other specialized trainings as identified
• Assure timely and accurate documentation

• Must be at least 18 years old.
• Experience working with or knowledge of children with special health care needs preferred.
• Must be willing to provide transportation as a ChildServe authorized driver if required. This includes passing a motor vehicle record check and car safety check and able to provide a valid driver’s license and insurance.
• Good oral and written communication skills and have the ability to accurately document all observations, significant events, and services provided.
• Ability to communicate effectively in reading, writing, and speaking the English language
• This position may be subject to vaccination safety requirements. If applicable, specific requirements will be discussed during the interview process.
