Description

We are looking for a dedicated RN Care Coordinator to join our home care team. In this role, you will be responsible for assessing, planning, and managing the clinical care of children, ensuring their needs are met through a multidisciplinary approach. As an integral member of our team, you will work alongside the Clinical Supervisor to meet all federally regulated Conditions of Participation, developing and implementing individualized care plans in collaboration with families and the clinical team.

Schedule 
 
  • Monday-Friday
  • 8:00 am-4:30pm 
What You’ll Do    
  • Manage care for a caseload of children, assessing their health needs every 60 days, and developing care plans in accordance with physician orders and regulatory requirements.
  • Facilitate care meetings with families and the multidisciplinary team upon admission and every 60 days thereafter.
  • Ensure the implementation and documentation of each child’s care plan, maintaining compliance with agency, licensure, and accreditation standards.
  • Monitor services provided and maintain ongoing communication with families and team members to update care plans as necessary.
 
What You’ll Need
  • Licensed Registered Nurse in the State of Iowa. 
  • Preferred experience in developmental disabilities, pediatrics, or skilled patients.
  • Pediatric Nursing Certification or the ability to obtain it within two years of hire.
  • One year of home health care experience preferred.
  • Two years of supervisory experience or three years of care coordination instead of supervisory experience is preferred.
  • Ability to lift up to 35 lbs, be mobile within the clinic areas, and frequently bend, lift, reach, and stand,
  • Active BLS from the American Heart Association or American Red Cross
  • Ability to communicate effectively in (or through) reading, writing, and speaking the English language
