The Nurse Practitioner, reporting to the CMO and working under the supervision of a PCP, provides medical services to a diverse, frail and elderly population at one or more of CEI's primary care clinics and is a critical part of an Interdisciplinary Team (IDT) operating under the "PACE" model.  The IDT is comprised of individuals that include social workers, behavioral health specialists, nurses, physicians, activity directors, physical therapists, occupational therapists, recreational therapists, dietitians, and, transportation and related support staff. 

 
  

The salary range for the NP role at Center For Elders Independence is $ 121,000 - $ 193,000. Wages/salary are based on the market for the NP position, as well as experience, skills, abilities and work history  

   

          

Duties and Responsibilities:  

  • Acting within the scope of practice; diagnose and treat acute health care problems including illnesses, infections and injuries, provides urgent care for acute medical conditions and may order and/or perform diagnostic tests.
  • Provides routine healthcare to participants, including conducting semi-annual and periodic assessments of participants and furnishing/ordering medications and devices.
  • Consults with staff physician as outlined in the Nurse Practitioner Protocol and organization’s standardized policy and procedure for nurse practitioner. 
  • Analyzes and interprets participant histories, symptoms, physical findings and diagnostic information to develop appropriate diagnosis.
  • Develops treatment plans based on scientific rationale, standards of care and professional practice guidelines and participates in IDT development of comprehensive care plans.
  • Counsel’s participants on drug regiments, possible side effects, interactions with other substances, interventions to modify behavior associated with health risks and may offer counsel on how best to follow the care plan.
  • Communicates care needs and expectations to outside consultants, nursing home staff, board & care facilities, and caregivers.
  • Documents care provided in the participant medical records in a timely fashion and according to policies and procedures.
  • Provides guidance to clinic nursing and support staff.
  • Performs related duties as assigned.

   

   

 Qualifications:  

  • Masters Prepared Nurse Practitioner—Family, Adult, or Geriatric certificate and licensed as a Registered Nurse in California. Must be certified by the BRN.
  • Minimum of one year of experience working with the frail or elderly
  • DEA Certificate and Furnishing License required
  • Experience and interest in working in geriatrics.
  • A current certification in CPR.
  • Demonstrated ability to work effectively as a part of an interdisciplinary team.
